Obituary of Matthew John Chubak

Matthew John (Matt) Chubak of Calgary passed away on January 13, 2009 at the age of 20 years. Matthew was born on June 2, 1988 in Calgary, Alberta. Matthew is survived by his mother Sandy Chubak and step father Peter Mercer; sister Jocelyn Chubak (Luis Ordenez) and their son Domenic; grandmother Evelyn Chubak; aunt Cindy Chubak; uncles Jeff Chubak and Scott Chubak; cousin Brandi Hudgin (Michael) and their children Chloe and Jacob; cousin Marley Hudgin (James) and their children Vienna and Akeria; cousin Vanessa Hudgin; aunt and uncle Cheryl and Calvin Solvey; cousin Sara Solvey (Stewart); cousin Kristen Solvey and cousin Kenton Solvey. Matt was predeceased by his father Nathan Horsman and his grandfather John Chubak.
